As a community-driven nonprofit, we walk alongside the people we serve to build a North Carolina where everyone has the opportunity to live a healthy life and access care. Each of our programs specializes in a distinct yet interconnected area of health. Our unique organizational structure fosters collaboration across disciplines and cross-sector partnerships to advance whole-person, whole-community, and whole-state health.

Center of Excellence for Integrated Care

The Center of Excellence for Integrated Care (COE) supports clinics and systems across North Carolina in transforming their operations to provide whole-person care through workforce development, practice transformation, and advocacy.

Jim Bernstein Community Health Leadership Fellows

Bernstein Fellows are the future of health leadership in North Carolina. The fellowship supports outstanding individuals who share our founding director Jim Bernstein’s passion for rural health. They work alongside rural and underserved communities to identify actionable solutions to pressing health challenges.

Jim and Sue Bernstein Health Center Scholarships

Jim Bernstein established a network of 81 community-based clinics and centers across North Carolina. He and his wife Sue, a former Peace Corps volunteer, created the Jim and Sue Bernstein Health Center Scholarships. Awarded annually, the scholarship pays for the clinical education of employees working at eligible health centers and college for their dependents.

NCCARE360

NCCARE360, the first statewide coordinated care network, connects North Carolinians with vital social support, from housing and transportation assistance to food access and more. It links providers and organizations through a shared technology platform, creating a closed-loop referral system that provides a solution to a fragmented health and human services system.

North Carolina Oral Health Collaborative

The North Carolina Oral Health Collaborative (NCOHC) seeks to resolve barriers to good oral health and accelerate policies that improve oral health for all North Carolinians. Through advocacy, programmatic work, and resource development, NCOHC aims to build a future where everyone has access to the oral health care they deserve.

North Carolina Rural Health Association

The North Carolina Rural Health Association (NCRHA) advocates for access to fair, affordable health care for everyone in North Carolina, regardless of who they are or where they live. The official state affiliate of the National Rural health Association, NCRHA is a network of associations, organizations, and individuals representing health care, education, economic development, local government, and more.

Project ECHO Network in North Carolina

Project ECHO Network in North Carolina (PEN-NC) connects Project ECHOs across the state. By coordinating across regions and subject areas, PEN-NC helps increase efficiencies and expand access to Project ECHO content and expertise, supporting health providers to deliver better care.
